About The Position

In this role, the pharmacist provides exceptional care to patients undergoing cardiac procedures or receiving cardiology care in our cardiology intensive care unit or general cardiology units. Decentral clinical services consist of collaborative practice with multidisciplinary teams, identification of drug-related problems, anticoagulation management and stewardship, pharmacokinetic monitoring, provision and documentation of recommendations through progress notes regarding optimization of drug therapy, multidisciplinary team rounding, admission and discharge medication reconciliation, transition of care planning, and direct patient care.

Requirements

Doctor of Pharmacy (Pharm.D.) degree
Completion of an ASHP-accredited PGY-1 Pharmacy Residency
Licensed or eligible for licensure as a pharmacist in Minnesota
Board Certification in Pharmacotherapy (BCPS) or eligibility and commitment to obtain within two years of hire
