Addison Gilbert Hospital’s Senior Adult Unit is seeking a Licensed Independent Clinical Social Worker (LICSW) who is passionate about advocating for older adults and guiding patients through meaningful, therapeutic transitions in care. This is a highly collaborative role where your clinical judgment, leadership, and compassion directly impact patient outcomes. On our Senior Adult Unit, patients often face complex medical, emotional, and social challenges. You’ll play a critical role in coordinating care, supporting recovery, and ensuring safe, thoughtful transitions, all while working within a supportive, interdisciplinary team that values your expertise.

Requirements

Master’s Degree in Social Work or Mental Health Counseling
Current Massachusetts LICSW required
Prior clinical leadership experience or demonstrated leadership skills
Strong skills in: Individual case management & managed care
Motivational Interviewing and short‑term treatment models
De‑escalation and restraint management techniques
Psychosocial assessment and treatment planning
Brief family interventions
Highly organized, adaptable, and comfortable in a fast‑paced inpatient environment

Responsibilities

Plan and deliver safe, therapeutic treatment for patients on the Senior Adult Unit
Conduct ongoing psychosocial and case management assessments
Advocate for patient needs and communicate effectively with the treatment team
Identify and address barriers to discharge and treatment transitions
Coordinate referrals and collaborate with families, insurers, and community providers
Facilitate individual and group therapeutic interventions to support recovery and prevent relapse
Educate patients and families on illness, treatment options, and recovery pathways
Maintain accurate documentation and clinical records
Participate in clinical and administrative meetings that support quality patient care
Work closely with reimbursement partners to optimize high‑quality, patient‑centered care
