About The Position

The Charles Lea Center is seeking a full-time Innovation Coordinator to support people in living more effective and meaningful lives by helping them secure nontraditional services. This role involves working with individuals to access residential, employment, and community supports, utilizing a combination of staff support and enabling technologies. The Innovation Coordinator will focus on assessment, person-centered planning (including MAPs), linking individuals to services, training, and advocacy. This position is crucial for developing and implementing a new service in Tennessee aimed at assisting young adults through a person-centered, employment-first, and technology-first approach. The ideal candidate will be grounded in person-centered thinking and foster a creative, forward-thinking approach to service delivery.

Requirements

  • At least 18 years of age
  • Must be able to effectively read, write, and communicate verbally in English
  • Ability to pass a background check upon hire
  • Valid driver's license
  • Vehicle Insurance
  • A bachelor’s degree from an accredited college or university in a human services field; or
  • A bachelor’s degree from an accredited college or university in a non-related field and one year of relevant experience; or
  • Associate degree and 2 years of relevant experience; or
  • High School Diploma or General Equivalent with 4 years of relevant case management experience providing or coordinating services to people with disabilities and special needs and/or with case management services.
  • Relevant experience means experience working directly with persons with intellectual, developmental, or other types of disabilities or mental illness.
  • Ability to work independently as well as with a team
  • Good work ethic and professionalism
  • Genuine desire to help people and build a career on doing good.

Responsibilities

  • Support people to live more effective and meaningful lives by working with them to secure nontraditional services that encompass a combination of staff supports and enabling technologies.
  • Facilitate access to residential, employment, and community supports.
  • Conduct assessments and person-centered/MAPs planning.
  • Link people to services.
  • Provide training.
  • Engage in advocacy on behalf of individuals and their guardians.
  • Provide technical assistance to Community Providers implementing services for people referred.
  • Advocate on behalf of person / guardian to assure that services being provided reflect the desires of person guardian.
  • Conduct 30, 60, 90 day follow-ups upon successful transition to community services to ensure satisfaction and achievement of desired outcomes.
  • Follow the standards and regulations required by Tennessee Department Disability and Aging (DDA).
