About The Position

This position is responsible for coordinating injury prevention initiatives, particularly focusing on elder falls. The role involves creating and managing an Injury Prevention Task Force, developing program goals and protocols, facilitating meetings, and ensuring the sustainability of grant-funded programs. The coordinator will also be responsible for training staff and community members on evidence-based practices, developing marketing campaigns, and managing program evaluation and data collection. Additionally, the role involves significant grant administration, including preparing applications, submitting reports, monitoring expenditures, and ensuring compliance with federal regulations.

Requirements

  • Valid, unrestricted insurable driver’s license.
  • Three (3) years of direct work experience in healthcare and/or prevention.
  • Bachelor’s degree in Health Sciences or related field or equivalent experience.
  • Resumes and references are required.
  • Must be an enrolled member of the Navajo Nation or other federally-recognized Indian tribes for preference.

Responsibilities

  • Creating, recruiting, and assembling an Injury Prevention Task Force in collaboration with relevant stakeholders to address elder falls.
  • Developing and maintaining positive relationships with tribal leadership, organizational leadership, FDIHB departments, community program partners, and task force members.
  • Developing the purpose, goals, operational logistics, objectives, scope, benchmarks, protocols, policy, communication plan, and membership for the Injury Prevention Task Force.
  • Facilitating task force meetings, monitoring attendance and events, observing activities, and reporting results.
  • Developing a sustainability plan with the Injury Prevention Task Force to ensure program continuation after grant funding ends.
  • Attending and obtaining master certification in an evidence-based practice within one year of training, and facilitating trainings for FDIHB staff and community partners.
  • Coordinating with trained facilitators and program partners to schedule evidence-based classes in the community.
  • Traveling periodically to communities and other functions to facilitate classes, attend tribal injury prevention initiatives, and observe program activities.
  • Establishing and maintaining a program information folder on FDIHB’s share drive for data and results.
  • Working with the Wellness Department to collect, summarize, and report participant assessment data.
  • Developing a marketing campaign with the Marketing team to spread prevention awareness and recruit participants for the grant-funded program.
  • Developing and maintaining an evaluation plan for project data collection, including baseline, timeline, and outcomes, and monitoring progress.
  • Generating a data collection plan for program evaluation and creating necessary tools, such as surveys.
  • Participating in all required funder conference calls, in-person meetings, trainings, and/or site visits.
  • Preparing the TIPCAP federal grant application and continuation applications.
  • Submitting required timely progress reports.
  • Complying with Uniform Administrative Requirements, Cost Principles, and Audit Requirements for Federal Awards and attending training for ethical compliance.
  • Monitoring, approving, reconciling, and reviewing all grant expenditures to ensure costs adhere to federal requirements.
  • Confirming with the FDIHB Finance team that grant budgets in the accounting system reflect the Notice of Award.
  • Assisting the Finance team in preparing for internal and external audits.
  • Overseeing the transition of programmatic changes and informing supervisor and funder as necessary.
  • Performing other duties as assigned.
