About The Position

Soleo Health is seeking a full-time Infusion Nurse to provide primary patient care to assigned patients receiving infusion therapy or related nursing services in their homes or alternate care settings in Greenwood Village, Colorado, and the immediate surrounding area. The nurse performs assessments, care planning, evaluation, and patient education in accordance with care matrices and best practices. The role is scheduled for 40 hours per week, generally Monday through Friday from 8:30 a.m. to 5:00 p.m., with flexibility for occasional evenings and weekends. The nurse participates in an on-call rotation and must be available to respond to emergencies during non-standard hours, including nights, weekends, and holidays, as required. The position involves driving up to one hour to visit patients; mileage reimbursement and paid drive time are provided. Soleo Health is a national provider of specialty pharmacy and infusion services delivered in the home or at alternate sites of care. Requirements

Current RN license issued by the applicable state Board of Nursing, in good standing; the role requires licensure as an RN in Colorado.
At least 3 years of nursing experience.
Knowledge of and experience using IV access devices and administering infusion medications, including peripheral IVs, central lines, PICCs, port access, and infusion pump troubleshooting.
Ability to drive to patients’ homes for visits, including travel of up to one hour.
Comfort administering infusions in patients’ homes.
Proficiency with Microsoft Outlook and Microsoft Office Suite.
Valid state-issued driver’s license with no significant restrictions.
Availability for the stated schedule, occasional evenings and weekends, on-call rotation, and emergency response during non-standard hours as required.

Nice To Haves

Bachelor of Science in Nursing (BSN) from an accredited institution.
Advanced certification in a specialty area of practice, such as CRNI, ONS, or IgRN.
Nursing experience with a background in infusion therapy.
Experience in ICU and/or cardiac care.
Familiarity with AlayaCare, Insight, CPR+, Concur, and Paylocity.

Responsibilities

Provide direct care to assigned patients and maintain communication with care team members, including pharmacists, prescribers, dietitians, care partners, and relevant community resources.
Evaluate and educate patients and care partners on safely administering home infusion therapy, providing additional instruction when needed.
Assess the home environment and identify interventions needed to make it safe and appropriate for prescribed care.
Collaborate with team members to develop plans of care and evaluate patients’ responses to therapy at each encounter.
Manage schedules and coordinate patient care using company-approved platforms, and promptly communicate scheduling changes to appropriate team members.
Participate in the nurse on-call rotation to provide continuous nursing support.
Document patient encounters, communications, and care activities at the time of service in accordance with company policy and regulatory requirements.
Maintain knowledge of Soleo therapies, protocols, and clinical standards.
Submit plans of treatment and required clinical documents on time, in accordance with policy and state guidelines.
Coordinate with relevant departments to complete and return required patient documents, including consents and plans of treatment, to the branch office within an acceptable timeframe.
Provide administrative oversight by monitoring workflows, ensuring policy compliance, and managing resources efficiently.
Respond promptly to emergencies to support rapid and effective issue resolution.
Perform other duties as assigned.
