About The Position

Soleo Health is seeking a per diem Infusion Nurse to provide primary patient care for assigned patients receiving infusion therapy and related nursing services in their homes or alternate-site settings in Longmont, Colorado, and the immediate surrounding area. The nurse performs assessments, care planning, evaluation, and patient and care-partner education in accordance with care matrices, best practices, company policies, and regulatory requirements. The role involves direct patient care and coordination with pharmacists, prescribers, dietitians, care partners, home patient representatives, and community resources. The nurse evaluates the patient’s home environment for safety, develops and evaluates plans of care with the care team, coordinates scheduling and care through company-approved platforms, and documents encounters and communications at the time of service. The nurse also submits required plans of treatment and clinical documents, maintains knowledge of Soleo’s therapies and protocols, and participates in the nurse on-call rotation. The position is per diem, approximately 10–20 hours per week, with variable scheduling and patient visits in the home.

Requirements

Current RN license issued by the applicable state Board of Nursing, in good standing; Colorado RN licensure is required.
RN degree.
Minimum 3 years of nursing experience; infusion therapy background preferred.
Knowledge and hands-on use of IV access devices and related skills, including IV lines, port access, peripheral IVs, central lines, PICCs, infusion medication administration, and infusion-pump troubleshooting.
Comfort administering infusions in patients’ homes.
Availability for at least 2–3 days per week to see patients.
Ability to drive up to one hour to patients’ homes.
Valid state-issued driver’s license with no significant restrictions.
Proficiency with Microsoft Outlook and Microsoft Office Suite.
Availability to respond to emergencies promptly, including during non-standard hours, nights, weekends, and holidays, as required.

Nice To Haves

Bachelor of Science in Nursing (BSN) from an accredited institution.
Advanced specialty certification, such as CRNI, ONS, or IgRN; preferred but not required.
Infusion therapy experience.
Experience in ICU and/or cardiac care.
Knowledge of AlayaCare, Insight, CPR+, Concur, and Paylocity.

Responsibilities

Provide direct care for assigned patients and communicate with appropriate care-team members and community resources.
Evaluate and educate patients and care partners on safe administration of home infusion therapy and provide additional instruction as needed.
Evaluate the home environment and identify interventions needed to ensure it is safe and appropriate for prescribed care.
Develop plans of care with other team members and evaluate the patient’s response to therapy at each encounter.
Manage the schedule through the company-approved platform and promptly communicate scheduling changes.
Coordinate patient care through the approved company platform.
Participate in the nurse on-call rotation to provide continuous nursing support.
Document patient encounters, communications, and care activities at the time of service in accordance with company policy and regulatory requirements.
Maintain knowledge of Soleo therapies, protocols, and clinical standards.
Submit plans of treatment and required clinical documents in a timely manner in accordance with policy and state guidelines.
Work with relevant departments to ensure required patient documents, including consents and plans of treatment, are completed and returned to the branch office within an acceptable timeframe.
Provide administrative oversight, including monitoring workflows, ensuring policy compliance, and managing resources efficiently.
Respond promptly to emergencies, including when required during non-standard hours, nights, weekends, and holidays.
Perform other duties as assigned.
