About The Position

Soleo Health is seeking a per diem Infusion Nurse to provide primary patient care to assigned patients receiving infusion therapy and related nursing services in their homes or alternate care settings in Parker, Colorado, and the immediate surrounding area. The nurse conducts patient assessments, participates in care planning, evaluates responses to therapy, and provides patient and care-partner education in accordance with care matrices and best practices. The role includes coordinating with the care team and community resources, assessing the safety and appropriateness of the home environment, managing scheduling and care coordination through company-approved platforms, and documenting patient encounters and communications. The nurse also participates in the on-call rotation, responds to emergencies as required, and ensures timely completion and submission of plans of treatment and other clinical documents. This is a per diem position averaging 10–20 hours per week. The schedule is variable and may include days, afternoons, evenings, and weekends; candidates must have at least two to three days of weekday availability. The nurse must be able to travel up to one hour to patients’ homes. Mileage reimbursement and paid drive time are provided.

Requirements

Current RN license from the applicable state Board of Nursing, in good standing; the posting specifies licensure as an RN in Colorado.
Minimum of 3 years of nursing experience; an infusion therapy background is preferred.
Knowledge and experience with IV access devices and infusion administration, including peripheral IVs, central lines, PICCs, port access, and troubleshooting infusion pumps.
Comfort administering infusions in patients’ homes.
Ability to drive up to one hour to patients’ homes and hold a valid state-issued driver’s license without significant restrictions.
Availability for at least two to three days during the week and ability to work a variable schedule, including days, afternoons, evenings, and weekends as needed.
Availability to participate in on-call support and respond to emergencies during non-standard hours, including nights, weekends, and holidays, as required.
Proficiency with Microsoft Outlook and Microsoft Office Suite.

Nice To Haves

Bachelor of Science in Nursing (BSN) from an accredited institution.
Advanced certification in a specialty area, such as CRNI, ONS, or IgRN.
Experience in ICU and/or cardiac care.
Computer knowledge of AlayaCare, Insight, CPR+, Concur, and Paylocity.

Responsibilities

Provide direct care to assigned patients and maintain communication with pharmacists, prescribers, dietitians, patients’ care partners, home patient representatives, and appropriate community resources.
Evaluate patients and educate patients or care partners on safe administration of home infusion therapy; provide additional instruction as needed.
Evaluate the home environment and identify interventions needed to make it safe and appropriate for prescribed care.
Develop plans of care with other team members and evaluate each patient’s response to therapy at each encounter.
Manage the schedule using the company-approved platform and promptly communicate scheduling changes to appropriate team members.
Coordinate patient care through the approved company platform.
Participate in the nurse on-call rotation to provide continuous nursing support to customers.
Document patient encounters, communications, and care activities at the time of service in accordance with company policy and regulatory requirements.
Maintain knowledge of Soleo therapies, protocols, and clinical standards.
Submit plans of treatment and required clinical documents on time, in accordance with policy and state guidelines.
Work with appropriate departments to ensure required patient documents, including consents and plans of treatment, are completed and returned to the branch office within an acceptable timeframe.
Provide administrative oversight by monitoring workflows, ensuring policy compliance, and managing resources efficiently.
Respond promptly to emergencies, including outside standard hours when required.
Perform other duties as assigned.
