The Infusion Intake Coordinator will directly educate a prospective patient through the intake process leading to enrollment with Always Compassionate Health. This person will work closely with referral sources, account managers, clinical team, and provider relations to ensure all tasks are efficiently completed associated with the enrollment process. The coordinator's direct job function would allow a patient to receive the proper care they need and confirm all information about said patient has been recorded correctly.

Requirements

Ability to work in a fast-paced environment and demonstrate ability to multi-task
Excellent organizational and time-management skills
At least 3-5 years of experience in a Home Care setting
Strong communication and interpersonal skills
Attention to detail with problem solving skills
Strong customer service background

Responsibilities

Educate a prospective patient through the intake process leading to enrollment with Always Compassionate Health.
Work closely with referral sources, account managers, clinical team, and provider relations to ensure all tasks are efficiently completed associated with the enrollment process.
Allow a patient to receive the proper care they need and confirm all information about said patient has been recorded correctly.
Triage new cases, present issues, and other factors impacting initial case assignments.
Manage acute and chronic therapies including IVIg, TPN’s, and antibiotics.
Collaborate with coordinators and field healthcare professionals to ensure seamless care delivery.
Respond to inquiries for outpatient referrals, gather demographic data, collect preliminary client information to identify the appropriate clinician, and assist in scheduling initial appointments.
