Infusion Homecare Registered Nurse (RN) 1:1 Weekly Infusion

Always Compassionate Health is a leading Licensed Home Care Services Agency dedicated to providing high-quality community home-based services with compassion and innovation. We are seeking an experienced and compassionate Infusion Homecare RN to provide weekly 1:1 IVIG infusion services to a patient in lower Putnam County. This role is crucial to our mission of enhancing the lives of individuals through exceptional home-based nursing care. The Home Infusion Nurse will collaborate with our Infusion Intake Team to ensure a smooth start to infusion care in patients' homes. We are looking for nurses who can contribute to our company's growth and expansion in home-based nursing services.
Requirements
- At least 3 years of full-time, or equivalent part-time, experience as a registered nurse in a home care infusion company, hospital, clinic, or medical facility.
- Infusion experience is REQUIRED.
- Must have experience infusing IV/SQIG and biologic medications.
- Knowledge of principles & practices of supervision, performance evaluation, and education.
- Knowledge of Federal HIPAA Regulations.
- Knowledge of standard nursing policies, procedures, methods, and techniques.
- Ability to read and interpret clinical documents, including physicians' orders and treatment plans.
- Ability to participate in a multi-disciplinary health care team.
- NYS Registered Nursing License and Registration Certificate.
- BLS necessary.
- Up-to-date medical records, inclusive of Physical, TB Clearance/QuantiFERON Gold blood test, 2-step PPD or T-Spot, or Chest X-ray and Habituation Statement.
- MMR-Varicella Titers Lab Report or Vaccination Records.
- TDAP Vaccine.
- Hep B Vaccination Series, Titers OR Declination.
- Urine Drug Screen (UDS) - within the last 6 months is acceptable.
- Flu Vaccination (if applicable) or Declination.
- Two (2) forms of government-approved ID to validate employment eligibility.
- Two (2) satisfactory professional references.
- Malpractice Insurance.
- Reliable transportation.
Nice To Haves
- Supervisory or managerial experience.
- ACLS preferred.
- Implanted port access (MediPort, PermaCath, etc.) competence is a plus!
Responsibilities
- Provide weekly 1:1 IVIG infusion services to a patient in their home.
- Partner with the Infusion Intake Team for seamless initiation of infusion care.
- Perform comprehensive nursing assessments.
- Manage PIV, PICC, Mid & Central line care and access.
- Access implanted ports (MediPort, PermaCath, etc.).
- Educate patients and/or families on therapy administration, including IV Infusion therapy preparation and administration.
- Ensure patient safety, monitor patients, and provide post-infusion follow-up.
- Communicate effectively with patients, families, the infusion pharmacy, and medical providers.
- Collaborate with the infusion manager, coordinators, and other internal office staff.
- Conduct on-site field visits to provide patient care.
- Supervise the competence of nursing staff upon request.
- Multitask and prioritize work responsibilities.
- Complete clinical documentation using a computer and tablet (paperless system).
