About The Position

Always Compassionate Health is a leading Licensed Home Care Services Agency dedicated to providing high-quality community home-based services with exceptional compassion and innovation. We are seeking experienced home infusion field nurses (RNs) for cases throughout Westchester and its surrounding areas. These nurses are critical to our direct patient care services, and we aim to grow and expand our home-based nursing services with their expertise. The role involves conducting patient visits to initiate care, performing follow-up visits, and potentially precepting new nurses. The Infusion Homecare Nurse will collaborate with the Infusion Intake Team for seamless care initiation in patients' homes.

Requirements

• At least 3 years of full-time, or equivalent part-time, experience as a registered nurse in a home care infusion company, hospital, clinic, or medical facility.
• Infusion experience is REQUIRED.
• Must have experience infusing IV/SQIG and biologic medications!
• Knowledge of principles & practices of supervision, performance evaluation, and education.
• Knowledge of Federal HIPAA Regulations.
• Knowledge of standard nursing policies, procedures, methods, and techniques.
• Ability to read and interpret clinical documents, including physicians' orders and treatment plans.
• Ability to participate in a multi-disciplinary health care team.
• NYS Registered Nursing License and Registration Certificate.
• BLS necessary.
• Up-to-date medical, inclusive of Physical, TB Clearance/QuantiFERON Gold blood test, 2-step PPD or T-Spot, or Chest X-ray and Habituation Statement.
• MMR-Varicella Titers Lab Report or Vaccination Records.
• TDAP Vaccine.
• Hep B Vaccination Series, Titers OR Declination.
• Urine Drug Screen (UDS) - within the last 6 months is acceptable.
• Flu Vaccination (if applicable) or Declination.
• Two (2) forms of government-approved ID to validate employment eligibility.
• Two (2) satisfactory professional references.
• Malpractice Insurance.
• Reliable transportation.
• Minimum 10 hours per week (home visit cases).

Nice To Haves

• Supervisory or managerial experience PREFERRED.
• ACLS preferred.
• Implanted port access (MediPort, PermaCath, etc.) competence is a plus!

Responsibilities

• Conducting patient visits to initiate patient care.
• Performing follow-up visits as needed.
• Precepting and evaluating the skills of newly hired nurses as requested.
• Partnering with the Infusion Intake Team to ensure seamless initiation of infusion care in patients' homes.
• Providing comprehensive nursing assessment skills.
• Performing PIV, PICC, Mid & Central line care, and access competence.
• Performing Implanted port access (MediPort, PermaCath, etc.) competence.
• Educating the patient and/or family in therapy administration via IV Infusion therapy preparation and administration.
• Ensuring patient safety, monitoring, and post-infusion follow-up.
• Communicating with patients, families, infusion pharmacy, and medical providers as necessary.
• Collaborating with the infusion manager, coordinators, and other internal office staff.
• Conducting on-site field visits to provide patient care.
• Supervising the competence of nursing staff upon request.
• Multitasking and prioritizing work responsibilities.
• Utilizing a computer and tablet for paperless clinical documentation.
