About The Position

Always Compassionate Health is a leading Licensed Home Care Services Agency providing high-quality community home-based services with compassion and innovation. We are seeking home infusion field nurses (RNs) with experience in various infusion therapies, venous access, and phlebotomy for cases throughout the Mid/Lower Hudson Valley (Orange/Rockland/Westchester/Putnam/Dutchess). RNs are critical to our healthcare services provided directly to clients. We aim to grow, adapt, and expand our home-based nursing services through the direct support, knowledge, skills, and expertise of our nurses. This role involves conducting patient visits to initiate patient care, performing follow-up visits, and potentially precepting and evaluating newly hired nurses. The Home Infusion Nurse will collaborate with the Infusion Intake Team for seamless initiation of infusion care in patients' homes.

Requirements

  • At least 3 years of full-time, or equivalent part-time, experience as a registered nurse in a home care infusion company, hospital, clinic, or medical facility.
  • Infusion experience is REQUIRED.
  • Experience infusing IV/SQIG and biologic medications.
  • Knowledge of principles & practices of supervision, performance evaluation, and education.
  • Knowledge of Federal HIPAA Regulations.
  • Knowledge of standard nursing policies, procedures, methods, and techniques.
  • Ability to read and interpret clinical documents, including physicians' orders and treatment plans.
  • Ability to participate in a multi-disciplinary health care team.
  • NYS Registered Nursing License and Registration Certificate.
  • BLS necessary.
  • Up-to-date medical, inclusive of Physical, TB Clearance/QuantiFERON Gold blood test, 2-step PPD or T-Spot, or Chest X-ray and Habituation Statement.
  • MMR-Varicella Titers Lab Report or Vaccination Records.
  • TDAP Vaccine.
  • Hep B Vaccination Series, Titers OR Declination.
  • Urine Drug Screen (UDS) - within the last 6 months is acceptable.
  • Flu Vaccination (if applicable) or Declination.
  • Two (2) forms of government-approved ID to validate employment eligibility.
  • Two (2) satisfactory professional references.
  • Malpractice Insurance.
  • Reliable transportation.
  • Ability to multitask and prioritize work responsibilities.
  • Time management skills.
  • Proficiency using a computer and tablet for paperless clinical documentation.

Nice To Haves

  • Supervisory or managerial experience.
  • ACLS preferred.
  • Implanted port access (MediPort, PermaCath, etc.) competence is a plus!

Responsibilities

  • Conducting patient visits to initiate patient care.
  • Performing follow-up visits as needed.
  • Precepting and evaluating the skills of newly hired nurses as requested.
  • Partnering with the Infusion Intake Team to ensure seamless initiation of infusion care in patients' homes.
  • Conducting comprehensive nursing assessments.
  • Performing PIV, PICC, Mid & Central line care, and access.
  • Performing implanted port access (MediPort, PermaCath, etc.).
  • Educating the patient and/or family in therapy administration via IV Infusion.
  • Preparing and administering IV Infusion therapy.
  • Monitoring patient safety and providing post-infusion follow-up.
  • Communicating with patients, families, infusion pharmacy, and medical providers.
  • Collaborating with the infusion manager, coordinators, and other internal office staff.
  • Conducting on-site field visits to provide patient care.
  • Supervising the competence of nursing staff upon request.
  • Multitasking and prioritizing work responsibilities.
  • Performing clinical documentation using a computer and tablet.
