About The Position

Always Compassionate Health is a leading Licensed Home Care Services Agency dedicated to providing the highest quality community home-based services with exceptional compassion and innovation. We are seeking experienced home infusion field nurses (RNs) for cases across Northern Manhattan, the South Bronx, and surrounding areas. Nurses are vital to our direct client care services, and we are looking for RNs whose expertise will help us grow and expand our home-based nursing services. This role involves conducting patient visits for care initiation, performing follow-up visits, and precepting new nurses. The Infusion Homecare Nurse will collaborate with the Infusion Intake Team for seamless patient care initiation in their homes.

Requirements

  • At least 3 years of full-time, or equivalent part-time, experience as a registered nurse in a home care infusion company, hospital, clinic, or medical facility.
  • Infusion experience is REQUIRED.
  • Must have experience infusing IV/SQIG and biologic medications!
  • Knowledge of principles & practices of supervision, performance evaluation, and education.
  • Knowledge of Federal HIPAA Regulations.
  • Knowledge of standard nursing policies, procedures, methods, and techniques.
  • Ability to read and interpret clinical documents, including physicians' orders and treatment plans.
  • Ability to participate in a multi-disciplinary health care team.
  • NYS Registered Nursing License and Registration Certificate.
  • BLS necessary.
  • Up-to-date medical, inclusive of Physical, TB Clearance/QuantiFERON Gold blood test, 2-step PPD or T-Spot, or Chest X-ray and Habituation Statement.
  • MMR-Varicella Titers Lab Report or Vaccination Records.
  • TDAP Vaccine.
  • Hep B Vaccination Series, Titers OR Declination.
  • Urine Drug Screen (UDS) - within the last 6 months is acceptable.
  • Flu Vaccination (if applicable) or Declination.
  • Two (2) forms of government-approved ID to validate employment eligibility.
  • Two (2) satisfactory professional references.
  • Malpractice Insurance.
  • Reliable transportation.
  • Ability to multitask and prioritize work responsibilities.
  • Time management skills are essential.

Nice To Haves

  • Supervisory or managerial experience PREFERRED.
  • ACLS preferred.
  • Competence in implanted port access (MediPort, PermaCath, etc.) is a plus!

Responsibilities

  • Conducting patient visits to initiate patient care.
  • Performing follow-up visits as needed.
  • Precepting and evaluating the skills of newly hired nurses as requested.
  • Partnering with the Infusion Intake Team to ensure seamless initiation of infusion care in patients' homes.
  • Conducting comprehensive nursing assessments.
  • Performing PIV, PICC, Mid & Central line care, and access.
  • Competence in implanted port access (MediPort, PermaCath, etc.).
  • Educating the patient and/or family in therapy administration.
  • Preparing and administering IV Infusion therapy.
  • Patient safety, monitoring, and post-infusion follow-up.
  • Communicating with patients, families, infusion pharmacy, and medical providers as necessary.
  • Collaborating with the infusion manager, coordinators, and other internal office staff.
  • Conducting on-site field visits to provide patient care.
  • Supervising nursing staff competence upon request.
  • Performing clinical documentation using a computer and tablet.
