About The Position

Toolworks is seeking a dedicated and compassionate Information & Referral Specialist to support our Aging and Disability Resource Center team. This is an opportunity to help people with disabilities and older adults access the services and supports they need to live independently and participate fully in their communities. In this full-time, in-person role, you will be based at our San Francisco headquarters and travel regularly throughout the Bay Area for community outreach, events, and client meetings. You will provide information, direct referrals, resource navigation, and follow-up support to people seeking services such as adult day programs, housing assistance, financial support, transportation, legal assistance, and health care resources. Your days will be varied and meaningful. You may begin the morning helping someone complete a housing application, spend the afternoon conducting outreach at a senior center, and finish the day documenting referrals and follow-up activities. You will also welcome visitors, assist with front-desk operations, and provide administrative and communication support as part of an inclusive and responsive team.

Requirements

  • At least two years of experience working with adults with disabilities, older adults, or other underserved communities, preferably in a community-based setting.
  • Strong organizational skills.
  • Strong interpersonal skills.
  • Strong written communication skills.

Nice To Haves

  • Master of Social Work degree.
  • Active California Associate Clinical Social Worker registration.
  • Bilingual abilities in American Sign Language, Spanish, or Cantonese.

Responsibilities
