About The Position

Community Care is hiring an Infection Prevention - Control Coordinator (RN), Part-Time, Day Shift. This role involves team collaboration, community-based work, and being part of a mission-driven organization focused on helping elderly individuals and adults with physical and intellectual disabilities live as independently as possible in the community. This is a full-time opportunity based out of our Brookfield/Bishops Woods office. The Infection Prevention - Control Coordinator, under the supervision of the Director of Clinical Services, is responsible for the ongoing development, coordination, and monitoring of Community Care’s health promotion and infection control programs for employees and program members. This role ensures compliance with all related governing regulations and leads and participates in clinical quality improvement projects and activities.

Requirements

  • Associate degree in Nursing
  • Wisconsin Licensed Registered Nurse required
  • CPR & AED required
  • WI Driver's License required
  • A minimum of 2 years of clinical nursing experience.

Nice To Haves

  • Bachelor degree in Nursing
  • Infection control certification preferred
  • One year of experience in infection control.

Responsibilities

  • Develops, implements, and monitors infection control programs, and assists with employee health programs, and policies and procedures.
  • Stays informed of trends, practices, and regulations related to health promotion and communicable disease prevention and management through research, continuing education and membership on applicable external associations.
  • Monitors policy compliance of clinical operations for employee and member TB screening, and oversees tracking of member screening.
  • Oversees disease outbreak investigation, management and reporting; acts as a liaison with Local/State Public Health Department.
  • Plans and coordinates annual influenza vaccine program for employees and members.
  • Acts as an educational health, wellness, and infection control resource for all employees.
  • Collaborates with the Learning and Organization Development staff to develop, provide or coordinate employee training, in-services, and continuing education.
  • Coordinates, monitors, reports on and ensures compliance of employee health/infection control.
  • Reviews nursing policies and procedures, and assists in the development of new clinical policies and forms.
  • Uses data to identify trends and implements strategies to improve clinical and operational outcomes. Monitors, audits and reports on nursing, clinic support and personal care service delivery and practices, and works with nursing leadership to resolve QI issues.
  • Develops, organizes and/or assists with continuing education for Nursing and Nursing Support functions.
  • Regular local travel - requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
