About The Position

Responsible for assessing surveillance documents daily, identifies “high-risk” diagnosis/lab reports, examines corresponding medical records for evidence of potential Hospital Acquired Infection, assembles data, and monitors staff response as evidenced by initiation of appropriate isolation. Conducts targeted surveillance to determine outbreaks, reports communicable, infectious diseases to local Board of Health, MD, nursing/ambulatory sites in designated time frame, enters demographics into CDRSS (Communicable Disease Reporting and Surveillance System), manages patient placement according to protocol. Researches and develops educational programs following established hospital policy, regulatory guidelines and accrediting agency requirements. Works collaboratively with direct care providers in achieving implementation of best practice and patient safety. Responsible to provide services to all sites within the healthcare system and associated physician offices and other tasks related to infection prevention and control as assigned.
