About The Position

This role focuses on providing specialized support to adult clients with severe and persistent mental illness to help them achieve competitive employment. The specialist will assess vocational needs, develop program plans, provide case management, and act as a liaison with the community to build a support network. This includes developing vocational resources, coordinating with clinical teams, and advocating for client needs. Extensive travel within Macomb County is required for client assessments and goal development. The role also involves networking with local employers, coordinating job fairs, and demonstrating knowledge of trauma-informed care principles.

Requirements

  • Experience in providing coaching to team staff on supported employment services.
  • Ability to assess and evaluate vocational needs of clients with severe and persistent mental illness.
  • Skill in developing program plans to meet identified needs.
  • Proficiency in completing referral forms and clinical/legal documentation.
  • Experience in maintaining records of client contact.
  • Ability to provide vocational case management services.
  • Experience monitoring and evaluating vocational situations and settings.

Responsibilities

  • Provides coaching to team staff on supported employment services, including strength-based interactions, person-centered planning, Stage of Change, Motivational Interviewing, and co-occurring principles.
  • Assesses and evaluates the vocational needs of adult clients with severe and persistent mental illness, develops goals, and creates appropriate program plans.
  • Completes necessary referral forms and clinical/legal documentation for community-based services.
  • Maintains records of client contact for agency documentation, team presentations, reviews, and staff meetings.
  • Provides vocational case management for support, rehabilitation, and crisis intervention.
  • Monitors and evaluates vocational situations and settings for treatment consistency and progress.
  • Coordinates with the clinical team to ensure client needs such as medication adherence, transportation, and basic needs are met.
  • Acts as a community liaison to establish a support network for client vocational needs and provides community education.
  • Advocates for new services when needed.
  • Seeks out and develops vocational resources, including relationships with local employers, to meet client needs for competitive employment.
  • May provide specialized training programs, community placement, and post-institutional follow-up services.
  • Travels extensively within Macomb County to assess client needs and develop program goals.
  • Attends local job fairs to network with employers and assist consumers in obtaining and retaining competitive employment.
  • Participates in support group functions.
  • Coordinates consumer job fairs and collaborates with outside vocational programs and agencies.
