Requirements

Associate degree in Human Services or a related field, or equivalent combination of education and experience
Experience in human services, case management, advocacy, education, healthcare, or a related field preferred
Strong communication, organization, and problem-solving skills
Ability to work independently while supporting individuals in community settings
Passion for inclusion, advocacy, and empowering others
Travel throughout New Hampshire is required
Pass criminal background check

Nice To Haves

Bachelor's degree in a related field
Experience working with individuals with disabilities or underserved populations
Familiarity with community resources and support services
Lived experience with a disability

Responsibilities

Meet with consumers in homes and community settings
Help individuals build independent living skills to achieve their personal goals
Connect consumers with employment, housing, and community resources
Support individuals through accessibility and home modification programs
Advocate alongside consumers to reduce barriers and increase opportunity
Maintain documentation and case notes
