About The Position

Pathway to Possible (P2P) is a congregate care facility providing independent living and assisted living for adults with developmental disabilities in the Newton and Wellesley areas. The program offers housing and support services for adults referred by the Department of Developmental Services. The program's goals include developing skills for productive participation in the residential program, fostering independence, and establishing lifelong connections. P2P is committed to providing a safe, healthy, and welcoming environment while ensuring resident safety.

Requirements

  • Bachelor's degree in human services or related field.
  • Commitment to the empowerment of individuals with intellectual and developmental disabilities.
  • Familiarity with the DDS delivery system.
  • A valid Massachusetts Driver's License required.
  • First AID and CPR Certified are required.
  • Proficient with Microsoft Office Suite or related software.
  • Legally authorized to work in the United States.
  • Clearance of CORI background checks as required by state and federal law.
  • Clearance of DPPC registry as required by state law.
  • A cleared search of finger printing through the DDS criminal background check.
  • Must be able to lift up to 15 pounds.
  • Must be able to provide physical assistance to adults using wheelchairs.
  • Must be able to evacuate individuals supported within 2 ½ minutes.
  • Must be able to assist individuals in transferring from sitting to standing.
  • Maintain MAP certification for the duration of employment.

Nice To Haves

  • A graduate degree and/or 5 years program management experience is preferred.
  • Current MAP certification preferred.
  • Experience with individuals who have complicated medical needs is preferred.
  • Clearance of SORI background checks as required by state and federal law.

Responsibilities

  • Oversee the overall operation of the Independent Living Program, ensuring high-quality services.
  • Schedule and supervise support staff.
  • Provide fiduciary oversight.
  • Oversee medical appointments and communication.
  • Perform administrative duties as required by P2P, DDS, and DPH.
  • Follow all P2P Personnel Policies and Procedures and Policies to Promote Human Dignity.
  • Provide regular documented supervision and feedback to case management staff, ensuring client safety and well-being.
  • Schedule and approve staffing to meet required coverage.
  • Provide up to 10 weeks of on-call coverage per year.
  • Actively participate in the management team, agency-wide events, staff meetings, and training.
  • Ensure all client health care and medical needs are met.
  • Maintain thorough knowledge of individuals’ financial resources, including government benefits, housing subsidies, and other entitlements.
  • Ensure client compliance with entitlement benefits.
  • Communicate and ensure implementation of all safety programs and policies, including maintaining accurate and current safety plans.
  • Ensure Individual Support Plans (ISP) are developed and implemented in accordance with P2P and DDS guidelines.
  • Ensure proper monthly documentation is completed.
  • Direct case managers to oversee the monitoring and scheduling of medical/dental appointments for each resident.
  • Participate in P2P and Department of Developmental Services training programs and attend all Program Manager meetings.
  • Monitor and coordinate with HMO for the proper operation of house, grounds, vehicles, furnishings, equipment, and physical plant.
  • Maintain all program records according to established guidelines and state and federal requirements.
  • Maintain financial accounts and budgetary guidelines in alignment with state, federal, and P2P guidelines.
  • Accurately document and report monthly billing based on hours of service received by each individual.
  • Provide the appropriate number of service hours for each individual based on their DDS contracts and individual needs.
  • Maintain relationships with involved families, guardians, and other providers.
  • Train ILP case managers in HCSIS, ICENTRIX, and client banking.
