About The Position
This application is intended for individuals who have been selected for a 1099 independent contractor opportunity with Quadrant Health Group and facilities. The purpose of this application is to collect the necessary information to initiate the contractor onboarding process. Specific details regarding your assignment—including scope of work, location, schedule, and compensation—will be outlined in your independent contractor agreement. Engagement is contingent upon completion of all required documentation, which may include background screening (if applicable), credential verification (if applicable), and execution of an independent contractor agreement.
