About The Position

St. John’s Community Services (SJCS) is seeking an Incident Management Coordinator to join the Quality Assurance Team. SJCS is a non-profit organization dedicated to supporting and advocating for people of all abilities, cultivating caring communities, and ensuring that all individuals have the opportunity to thrive. Requirements

  • Ability to gain thorough knowledge of St. John’s Community Services policies and procedures
  • Ability to act as a representative of St. John’s Community Services to the public
  • Thorough knowledge of program practices and procedures
  • Ability to develop and maintain a working knowledge of local, federal, and program regulations
  • Ability to maintain effective working relationships with associates, subordinates, representatives from other agencies, and the general public
  • Ability to effectively communicate with individuals from diverse backgrounds
  • Ability to effectively supervise, train, evaluate and communicate with program staff
  • Ability to research, prepare, review and maintain reports and documents
  • Ability to use sound judgment when making decisions
  • Ability to pass a criminal background check
  • Skilled in the operation of relevant computer systems, including hardware and software, and office machines
  • Able and willing to complete duties as directed and model professional behavior
  • Bachelor’s degree in a human services related field required
  • Two or more years’ direct service experience with adults with intellectual/developmental disabilities, preferably in a community-based program setting
  • Must meet the standards of a Qualified Intellectual Disabilities Professional (QIDP)
  • Must meet SJCS and funding jurisdiction current requirements for criminal background/records check
  • Obtain a valid health certificate to meet SJCS or current funding authority requirements as required
  • Meet SJCS and the funding authority requirement of criminal background and record checks

Nice To Haves

  • Master’s degree preferred
  • One-year supervisory experience preferred

Responsibilities

  • Management of DC Incident Management Coordination
  • Reporting GERs to MCIS for Reportable Incidents or Serious Reportable Incidents
  • Liaison for DDS IMEU Representative and maintain rapport and clear communication with DDS Quality Assurance and Improvement division
  • Ensures staff compliance with all acceptable incident management practices and procedures
  • Ensures that proper written documentation has been prepared for all unusual incidents
  • Ensures that incident reports are written by staff members by the end of their shift
  • Cooperates with investigation procedures as per state regulations
  • Reviews and responds to SJCS report card from DDS to ensure accuracy in reporting
  • Ensures compliance with notification requirements and timely submission to funding agencies
  • Trains staff on Incident Policy, Incident Reporting and required trainings as established by DDS Incident Management Division
  • Coordinate staff training events as delegated by DDS for the IMC
  • Attend DDS IMC monthly and as needed meetings
  • Complete Reportable Investigations reports, submit to DDS
  • Submit Incident Recommendations to MCIS
  • Manage tracking and trending for incident reports
  • Follow up on Incident Recommendations for investigations
  • Coordinate with QA to create internal Incident Management tools, processes, and procedures
  • Coordinate with QA for Corrective Action Plans of Incidents
  • Staff training for Incident Management to include NEO, ongoing training
  • Other job duties as assigned
