About The Position

Provides people in their own home with support services designed to promote self-determination, independence, productivity, integration and inclusion in the community. May also provide job coaching services out of own or family home. As an IHS Support staff, you will be expected to work independently to assist people with any and all areas of their life in which they require assistance, maintain communication with family or guardian regarding support, and provide transportation as needed using your personal vehicle. You will be required to work a flexible schedule based on the needs of the person or people you are working with.

Requirements

  • Must have at least a High School Diploma or its equivalent
  • Must have at least 3 years driving experience with a valid driver’s license
  • Behavioral Management Experience and/or formal training
  • Ability to communicate, both orally and in writing
  • Will be required to be PMT, CPR, FA and complete DDS Medication Administration Program (MAP)
  • Must be free from communicable disease

Responsibilities

  • Assists the people we support with activities of daily living, which may include but is not limited to: personal hygiene/self-care tasks, dressing, cleaning, cooking, budgeting.
  • Provides therapeutic environment.
  • Encourages participation; is helpful, supportive and respectful of differences with regards to unique needs, age, gender and culture of the person being supported.
  • Documents goal progress daily.
  • Assists people to participate in health related activities as required according to Journey Found and/or DDS policy, communicate all health needs with guardian.
  • Implements formal and informal skill building programs to help people learn new skills and increase independence.
  • Implements formal Behavior Support Plan (BSP) with support from the clinical team, where applicable.
  • Applies crisis prevention and intervention strategies, as approved by Journey Found.
  • Manages individual people in their home and in the community.
  • Works effectively with one or more people, encouraging participation in all programs and activities, providing support and offering assistance.
  • Provides transportation for people supported according to Journey Found transportation policies.
  • Must maintain minimum insurance coverage and be able to provide documentation of such coverage upon request.
  • Promotes a favorable image of the people supported and the organization by fostering good community relations by demonstrating appropriate professional behavior.
  • Must be able to work independently and work closely with guardian/family members.
  • Documents and notifies management of any issues brought forth that effect the therapeutic care or environment.
