Requirements
- Hawaii Physical Therapist license
- One year of clinical experience
- Excellent communication skills
- Ability to work independently
- Reliable transportation
Responsibilities
- Conduct physical therapy evaluations and reassessments
- Create treatment plans tailored to patient goals
- Improve patient mobility and safety
- Educate patients and families on exercise and fall prevention
- Coordinate care with the interdisciplinary team
- Maintain Medicare-compliant documentation
