About The Position

This role involves managing and coordinating referrals to community and government services to ensure comprehensive support for patients with complex Health-Related Social Needs (HRSN) and providing direct teaching and/or mentorship to Jefferson staff and students.

Requirements

Associate’s Degree in Public Health, Healthcare Administration or a related field OR Bachelor’s Degree in Public Health, Healthcare Administration or a related field
3 years experience in healthcare or benefits assistance, particularly direct patient care, public health, social work, case management or customer service
Proficiency in use of Microsoft Word, Excel, Outlook
Proficiency in data entry and familiarity with electronic health record systems
Ability to utilize resources effectively to overcome challenges and find solutions
Ability to multitask, problem-solve, and work in a fast-paced environment
Ability to stand or sit for extended periods
Ability to perform computer searches and telephonic outreach
Ability to work in hospital setting
Comfortable wearing personal protective equipment (PPE) as required by hospital guidelines
Requires travel to various on-site locations to conduct screenings, support on site staff/students, troubleshoot workflows, etc.

Prior experience working in a healthcare setting
Prior experience in community engagement or customer service

Responsibilities

Conduct HRSN assessments and other critical screenings onsite in various clinical and community settings for patients and community members.
Document screening results accurately and follow workflows to communicate positive screenings to relevant staff.
Identify and implement the best fit resources for each individual based on eligibility criteria such as medical history, insurance coverage, and screening results.
Engage directly with patients for clarification on needs and other relevant life circumstances.
Collaborate with care team members when resource coordination is directly relevant to patient care (e.g., medication costs / potential to change medications).
Provide training and oversight to students and staff on workflows and systems when supporting patients with Health Related Social Needs.
Facilitate learning through bedside teaching, case discussions, clinical demonstrations, and provision of constructive feedback.
