About The Position

PASCO is seeking an outgoing, resourceful IHSS Coordinator to champion our IHSS and Personal Care Services programs. This role is for individuals with experience in case management, Medicaid, or home healthcare who are eager for a bigger purpose. PASCO has been a leader in disability support in Colorado for over 30 years, and this position offers the chance to make a real impact in the lives of families, caregivers, and the community.

Requirements

Experience in Case Management or Client Care Coordination
At least 2 years of experience in client-facing roles, ensuring high levels of satisfaction.
Proficiency in Microsoft Office Suite, Outlook, and Electronic Health Record (EHR) systems.
Ability to work independently and in a fast-paced, evolving healthcare environment.
Willingness to Travel – Conduct home visits in the Lakewood area to ensure high-quality care.

Nice To Haves

Familiarity with Medicaid, Home Healthcare, or IHSS is a big plus!
Bilingual in Spanish (huge plus but not required)

Responsibilities

Work with case management entities to develop personalized care plans for PASCO clients, ensuring all needs are met.
Conduct on-site supervisory visits and provide feedback on Personal Care Workers (PCWs) and Homemakers.
Act as a trusted resource to help families navigate Medicaid and waiver services.
Work with PASCO departments, including Medical Records, Prior Authorization, HR, and Intake to streamline processes.
Keep records updated in Electronic Health Record (EHR) systems and ensure compliance with PASCO and state policies.
Establish strong community partnerships and act as a go-to support for clients, families, and caregivers.
