About The Position

The In-Home Therapist position accesses crisis needs and solutions to resolve immediate crisis. This position provides continuous monitoring of the status of clients being served. This position involves providing direct services to youth/young adults and their families. This employee is responsible for clinical and diagnostic assessments of client needs, will provide psychosocial interventions, crisis intervention and implement a community education/awareness program. This position is scheduled to work Monday-Friday, 8:00AM - 5:00PM.

Requirements

  • Master’s degree in social science
  • Two years post masters experience with SMI population preferred
  • Excellent verbal, written communication & planning skills
  • Knowledge of community agencies and resources
  • Knowledge of social security, Medicaid, & Department of Human Resources
  • Excellent 5-year motor vehicle driving record
  • Reliable transportation

Responsibilities

  • Work in frequent contact with co-therapist and medical personnel to provide mental health treatment in the community i.e. schools, home, etc.
  • Conduct intake assessments, CANS, and make referrals as identified from the assessments.
  • Provide individual, family, and group therapist for clients three times per week for a minimum of 12 weeks in the community.
  • Provide 24 hours availability to the clients in the program and community service organizations.
  • Assist clients in establishing & maintain a social support system/network within the community.
  • Complete necessary paperwork in a timely manner in accordance with the Center and/or DMH/MR standards.
  • Visit clients in their environment to assess progress towards goals, provide support & direction.
  • Advocate for the clients & assist client and family members in understanding the client’s diagnosis, traits, treatment approaches, etc.
  • Become aware & familiar with the client’s medications, possible side effects and conduct medication monitoring with client and family.
  • Relay information to co-therapist.
  • Perform crisis intervention and resolution as needed and/or required.
  • Work closely with Probation Offices to provide weekly reports on clients.
  • Participate in in-services, required meetings & trainings as mandated by supervisor.
  • Complete other duties as assigned.
