About The Position

Trillium Health Resources is seeking an IDD District Supervisor 1915i to oversee a skilled, integrated care team. This role involves providing localized support within a managed care environment to members with Intellectual or Developmental Disabilities or Traumatic Brain Injury who receive Non-Innovations Waiver funding. The supervisor will ensure staff implement industry-standard, evidence-based practices using the Trillium Connections Care Management platform. The goal is to ensure members receive whole-person care, addressing physical health, behavioral health, and social determinants of health to achieve positive outcomes and transform lives.

Requirements

  • Bachelor’s degree or licensure as an RN, and five (5) years of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI.
  • Master’s degree in a human services field or licensure as an RN, and three (3) years of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI.
  • Certification required if qualifying as a Registered Nurse.
  • Must have a valid driver’s license.
  • Must reside within Trillium’s Catchment of North Carolina.
  • Must be able to travel within catchment as required.

Responsibilities

  • Provide direct supervision and oversight for a localized, multi-disciplinary team supporting Innovations members living with mental health, substance use, and intellectual/developmental disabilities.
  • Ensure staff utilize workflows as required to complete Care Management comprehensive assessment and care plans for members as care needs change.
  • Establish a team-based, person-centered approach to coordination of care to be implemented by all team members that effectively manages a member’s physical health, behavioral health, and social determinant of health needs using established workflows and processes effectively and efficiently.
  • Monitor teams to help ensure integration of care through the establishment of a multi-disciplinary team to meet the member’s needs.
  • Be familiar with evidenced-based and best practices for members and translate that knowledge into specific process, procedures, and workflows for staff to identify and facilitate effective and efficient coordination and management of care for members.
