Easterseals MORC is hiring an IDD Care Coordinator for a community-based position serving Oakland County. The role focuses on coordinating individualized care and support, helping people access community resources, and promoting empowerment, choice, and satisfaction with services. Requirements

Bachelor’s degree from an accredited college or university, with a major in a human services field, in accordance with Medicaid Provider Manual Guidelines.
Valid Michigan driver’s license.

Responsibilities

Develop and monitor individual treatment and support plans, including accommodations for communication and choice.
Coordinate the Person Centered Planning process, evaluating progress, satisfaction, and safeguarding.
Provide counseling, education, and guidance to support empowerment, social skills, and relationship building.
Train caregivers to meet individuals’ needs and wishes.
Assist individuals in accessing community services and natural supports.
Help individuals select health care providers and manage financial resources.
Maintain contact with significant family members to obtain input and assess satisfaction with services.
