About The Position

The ICU Clinical Coordinator - Pharmacy works collaboratively with healthcare providers to optimize patient care. This role involves advising physicians on drug-related matters, promoting improved drug usage and therapeutic procedures, and coordinating pharmacy duties such as medication preparation, labeling, and delivery. The position ensures compliance with all relevant laws and regulations, involves patient observation and research to advance pharmaceutical care, and individualizes medication regimens based on patient-specific factors. The role also includes evaluating medication histories, assessing compliance, and suggesting modifications for desired outcomes.

Requirements

  • PharmD Degree required.
  • Pharmacist license required.
  • Three years of relevant work experience required.
  • Strong work ethic and professionalism.
  • Ability to work independently.
  • Excellent oral and written communication skills.
  • Knowledgeable in all pharmacy procedures and regulations.

Responsibilities

  • Works collaboratively with all health care providers to optimize patient care.
  • Advises physicians on the inherent dangers of drugs, incorrect prescription doses and their eventual side effects.
  • Promotes improvement in drug usage / dosage and in the supervision of therapeutic procedures for best results.
  • Coordinates the pharmacy duties, including the preparation, labeling and delivery of various medications or supplies according to prescriptions.
  • Ensures all dispensed drugs are recorded and delivered in accordance with local, state and federal laws or regulations.
  • Observes and treats patients in all health care settings and conducts research to further pharmaceutical care and advance the treatment and prevention of disease.
  • Individualizes medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption.
  • Suggests improvement in work methods.
  • Conducts and evaluates medication histories, assesses compliance and suggests modifications to achieve desired outcomes.
  • Performs other duties as assigned.
