The ICMS Case Manager provides intensive case management services to families experiencing homelessness or at risk of homelessness. This position assists eligible children (birth to age 18) and their families in obtaining and maintaining transitional and permanent housing while connecting them to medical, educational, social, pre-vocational, rehabilitative, and other community-based resources. Services are delivered through a Community-Based Scattered Site Model designed to promote family independence, minimize disruption, and support stronger, more sustainable housing outcomes. The ICMS Case Manager works collaboratively with families, housing providers, community partners, and multidisciplinary teams to ensure clients receive comprehensive, coordinated, and culturally responsive services.
