About The Position

Bridgeway Community Health is seeking qualified candidates for the Hybrid Stabilization Housing Aide / Peer Navigator position to support clients in their Women's Stabilization House and the Bridgeway Community Connect team. This role involves fostering a safe and sober environment, building positive relationships with clients and community partners, and supporting the agency's mission. As a Peer Navigator (mentor), the individual will collaborate with Care Coordinators, act as a first point of contact for new clients, assist with enrollment in services, and provide advocacy and education on addiction treatment resources. The position emphasizes a team approach to client care.

Requirements

  • Peer Support Specialist in Adult Addictions Certification (PSS) through Oregon Health Administration (OHA) OR Certified Recovery Mentor Certification (CRM) through the Mental Health and Addiction Certification Board of Oregon (MHACBO).
  • Typically requires a High School Diploma or General Education Certificate (GED).
  • Must possess current Oregon Food Handlers card or become certified within two weeks of hire date.
  • Must be CPR/First Aide certified or become certified within two weeks of hire date.
  • Must possess a current and unrestricted Oregon Driver's License for at least the last 3 years, with a clean driving record.
  • Qualified candidates must be able to pass a criminal background check.
  • Personal experience with addiction and recovery processes.
  • Must demonstrate knowledge of approaches to support others in addiction and recovery resiliency, competence in mentoring self-depicted recovery, and competence in mentoring and transition integration services for clients in the criminal justice system.
  • Excellent interpersonal communication skills and time management skills.
  • Ability to communicate clearly and concisely both orally and in writing, manage time and meet deadlines.
  • Understand and follow oral and written instructions.

Nice To Haves

  • In addition to a High School level education, preference is given to candidates enrolled in a college-level degree program and working towards gaining certification in a Human Services related field.

Responsibilities

  • Transports and accompanies clients to activities outside of the stabilization houses safely and prudently.
  • Assists in the day-to-day operation of the stabilization house by performing tasks as assigned by the program manager.
  • Develops and maintains a positive relationship with everyone served, provides monitoring, care, and support to clients in accordance with Bridgeway Community Health policies.
  • Assists and teaches individuals served to care for themselves in daily living tasks and social skills areas.
  • Prepares and serves meals as needed for holidays and events.
  • Conducts hourly bed checks if working overnight.
  • Accurately documents the support provided in the client's electronic health record.
  • Attends weekly team meetings.
  • Counts and records equipment and food items for inventory.
  • Maintains an organized and clean work environment.
  • Assists with urine analysis as needed to assure the ongoing safety and sobriety of the stabilization house.
  • Demonstrates recovery practices through role modeling, including pro-social thinking and behavior.
  • Provides day-to-day coaching on these practices to clients.
  • Assists clients in securing community resources and support.
  • Connects clients to other community resources such as 12-step support meetings, medical/dental services, etc.
  • Assists the treatment team in documenting the progress of the client from intake to completion of the program including coordination of services with criminal justice partners.
  • Facilitates mentor support groups or skills training groups.
  • Coordinates with team members and community partners to assist the care coordinator in developing specific treatment plans, interventions, strategies, and discharge planning.
  • Coordinates appropriate referrals and partners on follow-ups.
  • Collaborates with other service providers to ensure continuation of care.
  • Attends staff meetings as a team member to identify and implement quality of services.
  • Attends Bridgeway committee(s) as a program representative.
  • Assists/coordinates food inventory and ordering for the house.
  • Performs other duties as assigned.
