Requirements

Bachelor's Degree in Human Services such as Social Work, Psychology, Sociology or Counseling.
One (1) year of case management experience, including housing resources for adults living with mental health, developmental disabilities, substance use disorders, and co-occurring disorders.
Valid Driver's License for transporting clients as necessary.
Proficiency with various software and tools related to case management is essential, alongside solid documentation skills for maintaining accurate records in the Electronic Health Record (EHR) system.
Exceptional communication and interpersonal skills, enabling you to build rapport with clients and collaborate effectively with landlords and community partners.
Must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.

Responsibilities

Collaborate with clients to identify their unique housing needs and assist them in navigating housing opportunities, including the creation of comprehensive housing plans.
Manage a caseload, provide in-home, housing-focused skill supports, ensuring clients have the tools and resources necessary to secure stable, safe housing and achieve greater independence.
