About The Position
The Care Manager Associate provides coordination of health care and related services to infants, toddlers, school age children, adolescents, young adults, and their families for home & community-based services. Develops the plan of care with the client/family and interdisciplinary team and ensures client satisfaction with the services.
Requirements
- Case management experience in an outpatient setting.
- Bachelor's or equivalent in mental or public health related field required.
Nice To Haves
- 1-2 years Pediatric case management experience.
- Master's or equivalent in public health related field, criminal justice, psychology or related field.
