About The Position

This position involves planning, coordinating, implementing, billing, and monitoring case management services for HIV clients through the Department of Health – Alachua County for Area 3/13. The role includes performing medical case management functions for Ryan White (RW) clients according to state and federal regulations, documenting services in CareWare, and maintaining accurate client data. The specialist will also coordinate services with vendors and healthcare providers, collaborate with community organizations, and complete required reports.

Requirements

  • Valid driver’s license and access to reliable transportation.
  • Experience working in Electronic Medical Records.
  • Strong communication and customer service skills.
  • Ability to handle confidential information.
  • Must be a current Florida Department of Health employee.
  • Knowledge of Microsoft Systems.

Responsibilities

  • Plan, coordinate, implement, bill, and monitor case management services for HIV clients.
  • Perform medical case management functions for Ryan White (RW) clients as specified by current rules and regulations.
  • Document performed medical case management functions in CareWare, including progress notes, comprehensive needs assessments, comprehensive care plans, and acuity.
  • Ensure paper charts contain service authorizations, dental treatment plans, signed release of information, change of information forms, lab results, medical records, and correspondence.
  • Serve as back up for other Medical Case Managers.
  • Maintain accurate and current client data and information related to specific programs using approved state programs (HMS, ACCESS, Microsoft Office Word and Excel, CareWare).
  • Assist clinic personnel with activities to maintain clinic flow (faxing, making phone calls, etc.).
  • Ensure non-case managed clients in clinic are aware of Ryan White services.
  • Ensure required HRSA data is entered into CareWare, including client medications, CD4 values, Viral Load values, and documentation of HIV medical care.
  • Coordinate services between approved vendors, health care providers, clients, and/or client advocates.
  • Authorize services with approved vendors and submit appropriate paperwork for vendor services to the billing clerk by established deadlines.
  • Collaborate with community-based organizations and pharmacy personnel to ensure continuity of care.
  • Complete EARS weekly.
  • Attend and participate in staff meetings and training programs.
  • Perform other duties as assigned.
