About The Position

Protects and promotes the health of Richmond City residents by supporting chronic disease prevention initiatives that improve access to health education, community resources, and evidence-based prevention strategies. This is accomplished by planning, coordinating, and implementing chronic disease prevention programs; conducting community outreach and health education; developing and maintaining partnerships with community organizations, healthcare providers, and other stakeholders; connecting individuals to available health resources; and supporting community events and public health initiatives. Provides support for the Richmond and Henrico Health District's Community Health Improvement Plan (CHIP) by advancing chronic disease priorities through program planning, implementation, evaluation, and continuous quality improvement. Assists with identifying community needs, monitoring program activities and outcomes, maintaining program records and reports, and ensuring activities align with applicable federal, state, and local public health guidance, grant requirements, and agency priorities. Serves as a resource on chronic disease prevention by providing education, technical assistance, and outreach to community partners, organizations, and residents. Collaborates with internal and external stakeholders to strengthen partnerships, increase community engagement, and promote sustainable strategies that address health disparities and improve health outcomes. Provides support to the Health District and participates in additional public health initiatives, emergency response activities, and other duties as assigned to support the agency's mission.

Requirements

  • Knowledge of public health principles and practices related to chronic disease prevention, health promotion, community outreach, and health education.
  • Experience planning, coordinating, implementing, and evaluating community-based programs, projects, or public health initiatives.
  • Experience developing and maintaining collaborative relationships with community organizations, healthcare providers, and other stakeholders to support program goals.
  • Experience collecting, maintaining, and analyzing program data; preparing reports; and ensuring documentation is accurate, complete, and in compliance with applicable policies, procedures, or grant requirements.
  • Experience identifying community needs, analyzing problems, recommending solutions, and applying sound judgment to support program planning and continuous quality improvement.

Nice To Haves

  • Experience working in public health, population health, chronic disease prevention, health education, or community health programs.
  • Experience coordinating or supporting grant-funded programs, including grant reporting, program evaluation, or performance measure tracking.
  • Experience using public health data, community health assessments, or health improvement plans to inform program planning and decision-making.
  • Bilingual in Spanish and English preferred
