About The Position

This position is responsible for providing Case Management to pregnant women and infants referred to the Healthy Start Program. The Human Services Counselor I will establish a trusting relationship with culturally diverse families by initiating and maintaining regular client contact. This involves face-to-face contact with clients either in the OB provider’s office or in the client’s residence. The role includes providing Parenting Education, Breastfeeding Education, and Tobacco Cessation Education on a one-on-one basis with clients. All client contact information must be documented in the web-based computer application known as the Well Family System. Additionally, the position involves providing referrals to community resources to supplement Healthy Start services.

Requirements

• Experience in Counseling techniques and strategies
• Experience working with pregnant women and children.
• Problem solving skills related to advancing client self-sufficiency.
• Ability to work well with others as a team.
• Computer skills: competency and experience.
• Experience in interviewing and assessing to determine the needs of the client.
• Knowledge of community resources.
• Valid Florida Driver's License
• At least 1 year experience working with high-risk pregnant women and/or families
• Must have reliable and dependable transportation every day for home visitation purposes
• Ability to learn and communicate effectively, orally and in writing, in English.

Nice To Haves

• 1 to 3 years of experience working with families
• 1 to 3 years home visiting experience
• 1 to 3 years case management experience
• Associates Degree in a Social Service field
• Bilingual with fluency in English and Spanish
