About The Position

The City of Lakewood is hiring a full-time Human Services Case Manager (WRAP Specialist) for its Department of Human Services, Division of Youth. This role involves managing assigned cases, which includes conducting home visits, client assessment, developing service plans, making referrals, organizing meetings, providing crisis and disaster intervention, advocating for clients, facilitating support groups, collaborating with agencies, and assisting with client relocation and applications. The specialist will also meet with staff for case reviews, potentially speak publicly about programs, attend court hearings, and manage reports and files. A key aspect of the position is assisting in the planning and execution of County Contract deliverables, including mandatory training and meetings related to the Family-to-Family Neighborhood System of Care contract from the Cuyahoga County Division of Children and Family Services. The position offers an annual salary of $66,081.60 and operates primarily Monday through Friday from 8:00 am to 5:00 pm, with some flexibility and required weekend and evening hours. This is a county contract-funded position, renewed biennially, and its continuation is dependent on contract renewal.

Requirements

  • Bachelor’s degree from an accredited four-year college or university in a related field
  • Minimum of two (2) years of progressively responsible related experience working with at-risk Children, youth and families; or any combination of education, training and experience, which provides the required knowledge, skills, and abilities to perform the essential functions of the job
  • State of Ohio Social Worker License. (LSW) REQUIRED UPON HIRE
  • May be required to participate in testing at the discretion of the City
  • Must successfully pass a BCI&I fingerprint background check, drug screening and FBI background check if necessary

  • Preparing and submitting reports and related correspondence /documents and maintaining files
  • Assists in the planning and execution of all County Contract deliverables
  • Attend certain mandatory training sessions and meetings as outlined by funder including the Family-to-Family Neighborhood System of Care contract from the Cuyahoga County Division of Children and Family Services
