About The Position

The North Carolina Department of Health and Human Services (DHHS) is one of the largest, most complex agencies in the state, and has approximately 17,000 employees. It is responsible for ensuring the health, safety, and well-being of all North Carolinians, providing human service needs for special populations including individuals who are deaf, blind, developmentally disabled, and mentally ill, and helping poor North Carolinians achieve economic independence. This position is located in Franklin County, Louisburg, the satellite office of the Henderson Unit office. The Human Service Coordinator is responsible for providing basic casework duties to eligible consumers that have physical, mental, emotional, or learning disabilities to enable them to obtain and/or maintain competitive employment. Services are provided according to State, Federal and Agency policies and guidelines under the direct supervision of the Unit Manager or Rehabilitation Counselor Supervisor. The DHHS Division of Employment and Independence for People with Disabilities (EIPD) promotes employment and independence for people with disabilities through customer partnership and community leadership. EIPD helps people with disabilities achieve their goals for competitive employment and more independent living in communities statewide.

Requirements

  • Bachelor's degree in a discipline relative to the program served from an appropriately accredited institution; or Bachelor's degree from an appropriately accredited institution and one year of experience in an area related to program that provides experience in techniques of casework or counseling; or equivalent combination of education and experience.

  • Demonstrated ability to interpret medical and psychological data in planning and developing rehabilitative services

Responsibilities

  • Evaluating professional diagnostic data to determine eligibility and required services.
  • Developing and implementing individualized plans of rehabilitation services for eligible consumers.
  • Orchestrating and overseeing the provision of planned services.
  • Interviewing consumers for the purpose of assessing need.
  • Arranging for appropriate evaluations and other services indicated on the Individualized Plan.
  • Engaging in various administrative duties including case service documentation, caseload management, staff and other administrative meetings.
  • Participating in training and professional development activities.
