Catholic Charities Brooklyn and Queens, with over 125 years of service, offers 160-plus programs for various populations, including those struggling with mental illness through its Supportive Housing program. This program provides housing and mental health services to individuals with a history of homelessness, substance abuse, and chronic mental illness, offering counseling, case management, substance abuse rehabilitation, wellness, socialization, community integration, and crisis management. The Housing Wellness Coordinator is a key member of the Supportive Housing Programs for seniors, responsible for delivering on-site wellness services to address the behavioral health and social needs of residents. This role involves assessing building tenants, developing and implementing individual and group wellness activities to support aging in place, and providing supportive counseling and groups under the supervision of a licensed clinician. The primary goal is to help residents maintain permanent affordable housing and live with dignity, ensuring the Agency Mission and best practices are upheld.
