Catholic Charities Brooklyn and Queens, with over 125 years of service, offers 160-plus programs for children, youth, adults, seniors, and those with mental illness. The Queens Supported Housing Programs serve adults with severe mental illness requiring a DSM diagnosis and housing assistance. These programs are scattered site, meaning clients do not live in a single building. Under the direction of the Residential Supervisor, the OMH-Resident Wellness Coordinator provides rehabilitation and recovery services to residents in New York State Office of Mental Health (NYSOMH) funded Residential Programs. This role involves counseling, advocacy, and case management, ensuring residents are treated with respect and dignity. The coordinator implements best practices according to NYSOMH Part 595 Regulations and agency guidelines. Embracing an integrated system of care, the coordinator assesses client needs holistically and connects them to internal or external services, regardless of their entry point into the system.
