About The Position

This position is part of the Healthy Homes program in Manchester, CT, which is a rapid rehousing and scattered site housing program for individuals living with HIV or AIDS. The role involves providing direct case management, assisting clients with obtaining benefits, budgeting, medical services, and employment. It also includes linking clients to mental health and substance abuse services, providing financial assistance for security deposits and rent, and advocating with landlords. The specialist will also be involved in outreach and community education, maintaining awareness of the program, educating clients on tenant rights and responsibilities, and conducting outcome surveys. Documentation, participation in meetings, and additional duties as assigned are also part of the role.

Requirements

  • Bachelor’s Degree required, concentration in Human Services preferred.
  • 2 years’ experience working with homeless population or families dependent on specific position.
  • Valid driver’s license.
  • CPR/First-Aid Certification within 6 months of hire desired.

Responsibilities

  • Provide direct case management, including establishing rapport, writing treatment plans, teaching skills, and advocating for clients to obtain/maintain benefits, budgeting, medical services, employment, and transportation.
  • Assist clients in completing applications for Husky, Social Security, State Supplements, and other entitlements.
  • Assist clients with access to job and vocational training.
  • Link clients to appropriate mental health and/or substance abuse counseling and DCF services.
  • Provide direct assistance in the form of security deposits and time-limited rental assistance.
  • Advocate with landlords for tenant rights and provide conflict resolution.
  • Demonstrate competency in age-related or specialty issues and developmental needs for each population served.
  • Provide outreach/community education and act as a liaison with homeless service providers, Coordinated Access networks, DCF, domestic violence service providers, CHR providers, and courts.
  • Maintain awareness of the program within the community.
  • Assist in understanding tenant rights and responsibilities, as well as basic housing law.
  • Conduct outcome surveys as required.
  • Maintain documentation in compliance with agency and program standards.
  • Participate in staff meetings, supervision, trainings, and other meetings as assigned.
  • Perform additional duties as required.
