About The Position

This position is a temporary one year, (potentially renewable) contract to facilitate the Welcome Home Initiative, providing start-up assistance, life-skills coaching and on-going support for individuals and families transitioning into housing after experiencing homelessness or unstable housing situations.

Requirements

  • Willingness to develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities.
  • Work in compliance with OH&S Act and Regulations and abide by The Salvation Army’s health and safety policies and procedures.
  • Ability and willingness to undergo applicable screening and background checks successfully that are satisfactory to The Salvation Army, in its sole discretion.
  • Completion of a formal post-secondary/college diploma program of 2 academic years in Social Services or in a related field
  • Two years of prior related experience, including experience in a social service setting, supervision experience, experience working with marginalized individuals, and a social services background
  • Proficient in Microsoft Office (i.e., Word, Excel, Publisher, Access, and PowerPoint); the Internet and Email
  • Proficient in administration and files management procedures
  • Strong sense of integrity and confidentiality with professional ethics and a balanced sense of fairness and flexibility.
  • The incumbent will have good verbal and non-verbal communication skills.
  • Strong understanding of professional boundaries when working with a vulnerable population

Responsibilities

  • Completes intake, risk assessments and termination summaries as required; prepares and submits monthly, quarterly, and annual statistical reports to the ministry and The Salvation Army as required
  • Utilize best practices, develop case plans, utilize case management tools, report case notes, and develop strategy for clients within caseload, providing case management, case coordination, and various referrals.
  • Coordinates and facilitates life skills programming for program participants that includes, but is not limited to: financial literacy, employment skills, hygiene, health, food literacy, drug awareness, mental wellness, education planning, tenant’s rights, first aid, etc.
  • Provides appropriate follow-ups, monitors participant’s activities as per ministry, Salvation Army and funding standards
  • Assess client suitability to internal and external programs and agencies as required.
  • Ascertain and provide participants with referral services within the community and/or social services resources.
  • Ensures that programs are operated in accordance with the mission and values of The Salvation Army, and with applicable contract(s) and or agreements
  • Responds to program evaluations to adapt and improve program content
  • Liaises with Police, funders, court personnel, community agencies, government assistance programs, families, and other staff members
  • Sources and maintains program supplies, makes program purchases and arranges for delivery of goods
  • Keeps an accurate and up-to-date record of community resources for distribution to participants
  • Utilize knowledge of issues pertaining to the scope of program(s) to contribute to the development of program(s) materials and partnerships.
  • Contribute to the development of administrative frameworks and best practices for program delivery, including new and innovative approaches to programming and community involvement, creative approaches to crisis intervention and cross industry collaboration.
  • Ensure that program procedures are maintained in own work and when providing recommendations to others, support the Program Manager in developing relevant policies and present to the Leadership team, contribute to drafting of various learning guides and facilitator materials, where required.
  • Assist Management team in establishing meaningful connections with community partners, support and participate in stakeholder evaluations and focus groups, attend offsite meetings with stakeholders as required including sitting as active members on community committees.
  • Maintain appropriate records and statistics, ensures that all required documentation is accurate and complete.
  • Participate in training, meetings, case management development, administrative system orientations, as required.
  • Participate in test-run programming and utilize provided learning lab techniques to contribute to refining of the project concepts, materials, and delivery.
  • Performs other work-related duties as assigned.
  • Liaises with the participants’ supervising case manager, probation officer, social worker, or other community services to discuss participant’s progress
  • Ensures the physical and psychological needs of residents are being met through programming, goal setting and life skills coaching, etc.
  • Monitors participant’s progress and maintains relevant file information as per ministry or The Salvation Army requirements
  • Provides practical and emotional support and advocacy as required
  • Engages in the development of inter-personal relationships that promote dignity and respect
