About The Position

OCES is seeking two dedicated, caring Housing Specialists to work with its Housing team in new locations in Kingston, Carver, and Duxbury. The role involves working in the community and providing on-site services and support to residents. The Housing Specialist reviews residents’ needs and connects them with community programs and services, leads multidisciplinary assessment and service coordination, facilitates care conferences, and plans social, educational, and recreational activities. The role also prepares and distributes a newsletter to keep residents informed about planned activities.

Requirements

• At least two years of experience in the human services field, preferably working with the elder population.
• Passion for helping others and a desire to make a difference.
• Adaptability to changing consumer or program needs.

Nice To Haves

• Bachelor’s degree in Social Work or a related field preferred.
• Bilingual ability is a plus; Haitian Creole or Cape Verde Creole is preferred for one location.

Responsibilities

• Review residents’ needs and link them to community programs and services.
• Serve as the Multi-disciplinary Assessment Team Leader and meet with the team to ensure service coordination.
• Facilitate care conferences with residents, family members, and other participants.
• Plan and coordinate social, educational, and recreational programs for residents.
• Prepare and distribute a newsletter informing residents of planned activities.
