About The Position

OCES is seeking two dedicated, caring Housing Specialists to work with its Housing team and provide services and support to residents on-site at new locations in Kingston, Carver, and Duxbury. The role focuses on identifying residents’ needs and connecting them with community programs and services, coordinating multidisciplinary assessments and care conferences, and organizing social, educational, and recreational activities. The position offers a 35-hour work week, some flexibility, and a standard Monday–Friday schedule.

Requirements

• At least two years of experience in the human services field, preferably working with older adults.
• Passion for helping others and a desire to make a difference.
• Adaptability to changing consumer or program needs.

Nice To Haves

• A bachelor’s degree in Social Work or a related field is preferred.
• Bilingual ability is a plus; Haitian Creole or Cape Verde Creole is preferred for one location.

Responsibilities

• Review residents’ needs and connect them with community programs and services.
• Serve as the Multi-disciplinary Assessment Team Leader and meet with the team to ensure service coordination.
• Facilitate care conferences with residents, family members, and other participants.
• Plan and coordinate social, educational, and recreational programs for residents.
• Prepare and distribute a newsletter informing residents about planned activities.
