About The Position

CAMBA is a community organization dedicated to building an inclusive New York City by providing resources and support for individuals and families across various programs including Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. CAMBA operates shelters and drop-in centers, offering comprehensive services to help homeless individuals, often facing mental health and substance abuse challenges, transition into stable housing. The Flagstone Family Center specifically provides case management services to 160 families in a transitional residence program, housed in eight apartment buildings.

Requirements

  • High School Diploma or GED
  • Two years of relevant experience
  • Obtain State Central Registry (SCR) clearance
  • Obtain Register of Substantiated Category One Cases of Abuse or Neglect (Staff Exclusion List (SEL)) clearance
  • Obtain criminal history background checks via fingerprinting with the New York State Division of Criminal Justice Services
  • Submit medical Tuberculosis (TB) test results
  • Maintain all required clearances throughout employment

Responsibilities

  • Assist case managers by acting as a client liaison with outside organizations for matters related to education, healthcare, housing, social services, legal issues, and public benefits.
  • Help clients achieve their housing and entitlements goals by identifying and locating community resources and making referrals to services within and outside CAMBA.
  • Maintain professional relationships with clients and ensure client confidentiality.
  • Practice Universal Precautions/Standard Protocol & Procedures.
  • Comply with all Federal, State, City, and CAMBA security and privacy policies regarding individually identifiable health information.
  • Develop curriculum for shelter residents on Housing, Independent Living Skills, and Entitlement-related needs, including the permanent housing search process.
  • Develop and conduct monthly Housing readiness workshops.
  • Develop and maintain a Bulletin Board with relevant Housing information.
  • Provide individualized and group counseling on housing and entitlement topics.
  • Organize and schedule presentations by housing experts and landlords.
  • Provide support, training, and coordination for peer counselors.
  • Select appropriate instructional procedures (e.g., individual training, group instruction, lectures, role-play).
  • Establish linkages with community-based organizations, realtors, brokers, and landlords.
  • Conduct marketing and outreach presentations to recruit clients and resources.
  • Follow up with clients and referral organizations regarding client contact and progress.
  • Collaborate with case managers to help clients overcome barriers and advocate for themselves towards self-sufficiency.
  • Monitor client progress towards goals through regular contact and visits, documenting via progress notes.
  • Provide information for weekly, monthly, quarterly, and annual reports.
  • Create and maintain client files and make copies of client documents.
  • Assist case managers in scheduling client appointments with referral organizations.
  • Assist case managers in escorting clients to appointments (e.g., apartment viewings, public benefits, medical, social services).
  • Assist clients in completing applications for benefits and entitlements, and potentially process applications on their behalf.
  • Potentially follow up with clients after goal completion to ensure stability.
  • Potentially input client data and progress into an automated database.
