About The Position

This role is responsible for all aspects of an assigned caseload, including meeting with clients regularly, ensuring clinical files and reports are complete and up-to-date. The specialist will engage individuals using a client-centered approach, providing services and referrals for varied housing needs of individuals experiencing homelessness. Responsibilities include tracking trends in housing placements, orienting clients with the housing group, securing entitlements, maintaining correspondence related to housing interviews and vacancies, and assisting clients with coordinating moves with landlords and other internal/external agencies. The role also involves serving as a liaison with DHS and other providers to facilitate the housing process, providing housing retention services post-placement, participating in interdisciplinary team meetings and case conferences, and providing emergency first aid/CPR assistance when needed. Related duties are also assigned.

Requirements

  • High School Diploma/GED required.
  • Demonstrated writing, communication, and group facilitation skills.
  • Proficiency in MS Office, including Word and Excel.
  • Willingness to complete CPR certification training.

Nice To Haves

  • Bachelor's Degree preferred.
  • Significant experience working with individuals experiencing homelessness.
  • Experience with securing entitlements and housing.
  • Knowledge of mental illness and substance abuse.
  • Bilingual abilities.
  • CPR training certification.

Responsibilities
