About The Position

The mission of the CN Guidance FACT Team is to deliver comprehensive and effective services to individuals diagnosed with severe mental illness whose needs have not been well met by traditional service delivery approaches. The FACT Team provides an integrated set of evidenced-based treatment and support services through a highly individualized approach, with services delivered in the community in places and situations where problems arise.

Requirements

  • Bachelor's Degree in social work, psychology, rehabilitation counseling or related field required.
  • One year experience working in a mental health setting at the Masters or higher level or two years' experience working in a mental health setting at the Bachelors level.
  • Must have an understanding of concrete services i.e. Medicaid, Medicare, DSS and SPOA housing.

Responsibilities

  • Assist FACT Team recipients in obtaining and maintaining adequate housing.
  • Complete referrals and linkage to community benefits, organizations, and institutions that will assist the client in completing the stated treatment plan goals.
  • Responsible for Quality Improvement, Utilization Review and Performance Improvement activities including data collection, analysis of outcome trends, and identification of service provision needing improvement, completion of case records and timely submission of required documentation.
  • Plan and coordinate all services, in the office or in the field, which support and supplement the rendering of case management and care coordination services to clients, including applying for and maintenance of benefits through the Department of Social Services, Social Security Administration, housing applications, etc.
  • Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress.
  • Complete QI training during onboarding and participate in QI refresher training as needed.
  • Actively engage in agency-wide trainings and work groups, as well as NYS specific trainings on and for the FACT model.
  • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views, and experiences.
  • Adherence to all safety protocols and procedures to ensure a safe working environment for all employees.
  • Demonstrate a commitment to maintaining a safe work environment by following established safety guidelines.
  • Other duties as assigned.
