The Center for Human Development (CHD) is seeking a Housing Specialist for our Adult Community Clinical Services (ACCS), located in Springfield, MA. ACCS is funded through the Department of Mental Health and is a multidisciplinary outreach approach for persons served in need of psychiatric and other services living independently in the community. This involves active engagement including assertive outreach, consistent assessment and treatment planning, skill building, symptom management, addiction treatment, and behavioral and physical health monitoring and support. ACCS teams include licensed clinicians, RNs, housing specialists, substance use specialists, direct care staff, and peer/family specialists. The Team works collaboratively to provide person-centered, evidence-based services in support of Persons’ treatment goals. The Housing Coordinator with Adult Clinical Community Services (ACCS) participants to provide services to obtain and maintain permanent housing. The Housing Coordinator also provides outreach services to a caseload of participants. S/he works within the Agency's team model, establishing open communication, support, and accountability among co-workers.
