The Center for Human Development (CHD) is seeking a Housing Specialist for its Adult Community Clinical Services (ACCS) program in Springfield, MA. ACCS is funded by the Department of Mental Health and utilizes a multidisciplinary outreach approach for individuals needing psychiatric and other services while living independently in the community. This role involves active engagement, assertive outreach, consistent assessment and treatment planning, skill-building, symptom management, addiction treatment, and monitoring/support for behavioral and physical health. ACCS teams consist of licensed clinicians, RNs, housing specialists, substance use specialists, direct care staff, and peer/family specialists who collaborate to provide person-centered, evidence-based services aligned with participants' treatment goals. The Housing Specialist will work with ACCS participants to help them obtain and maintain permanent housing, providing outreach services to their caseload and operating within the Agency's team model to foster open communication, support, and accountability among colleagues.
