Requirements
- High School Diploma/GED required
- Significant experience working with individuals experiencing homelessness
- Significant experience with securing entitlements and housing
- Knowledge of mental illness and substance abuse helpful
- Demonstrated writing skills
- Demonstrated communication skills
- Demonstrated group facilitation skills
- Proficiency in MS Office, including Word and Excel
- CPR training certification or willingness to complete certification training in CPR
Nice To Haves
- Bachelor's Degree preferred
- Bilingual abilities a plus
Responsibilities
- Responsible for all aspects of assigned caseload, including meeting with clients on a regular basis and ensuring that all clinical files and reports are complete and up to date.
- Engaging individuals in a client-centered approach and providing services and referrals to address the varied housing needs of individuals experiencing homelessness.
- Tracking trends in housing placements and orienting clients with the housing group.
- Securing entitlements, maintaining correspondence relating to housing interviews and vacancies.
- Assisting clients to coordinate their move with landlords and other internal personal and external agencies.
- Serving as a liaison with DHS and other providers to facilitate the housing process for clients.
- Providing housing retention services once clients have been placed.
- Participating in interdisciplinary team meetings and case conferences.
- Providing emergency first aid/CPR assistance when needed.
- Related duties as assigned.
