About The Position

This position is responsible for the coordination and implementation of social services in the apartment complexes sponsored by Catholic Charities. The role involves providing general case management and referral services to residents, monitoring ongoing services, helping residents build support networks, and offering educational opportunities. The coordinator is also responsible for completing required statistics and reporting, collaborating with on-site management, maintaining resident files, and ensuring productivity outcome measures are achieved. Additionally, the position requires obtaining continuing education, maintaining community resource files, and adhering to HIPAA regulations for client information security. Communication via various channels and database practices are essential. As a mandated reporter, the coordinator must follow agency procedures for reporting child or adult abuse or neglect. Adherence to Catholic Social Teaching and Doctrine is expected.

Requirements

  • Demonstrated expertise; one-year residential setting experience preferred.
  • Demonstrated familiarity with community agencies and ability to access services.
  • Valid Driver’s License and ability to drive for work use.
  • Must successfully pass background check as applicable to position.

Responsibilities

  • Provide general case management and referral service to all residents requesting or needing assistance.
  • Monitor the ongoing provision of professional and volunteer services from Catholic Charities and other community agencies.
  • Help residents build informal support networks with other residents, families and friends.
  • Provide regular educational opportunities for residents.
  • Complete required statistics and reporting requirements re: CC, CQI, and HUD.
  • Collaborate with on-site management staff and volunteers.
  • Annually update, monitor, record in resident file tracking log, and document in progress notes all assistance, referrals, follow ups and interactions as they occur in resident files per CC, CQI, COA and HUD.
  • Performs as a team member to assure that productivity outcome measures are achieved.
  • Obtain a minimum of 12 hours per year in continuing education per HUD requirements.
  • Maintain and keep current with community resource files.
  • Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
  • Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor.
  • As a mandated reporter, follows all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect.
